- You must present a photo ID
- Drug screens are conducted on a first-come, first-serve walk-in basis- No appointments.
- No need to print out your authorization. We can accept it on your phone
- If your electronic authorization has expired, please contact your employer for an extension. We cannot process an expired authorization.
- Once the drug screening process begins, you must remain inside the facility until the process is completed.
- If you are unable to provide a sufficient sample, you will have 3 hours to provide an adequate amount to complete your screening. You will not be allowed to leave the building. 40 oz of water will be provided over the 3 hr period.
- At anytime during the collection process, if you become argumentative or hostile, you will be asked to exit the building, and your test will be entered as a “Refusal to Test.”
- Please bring all current medications and any secondary documentation from other medical providers. For example, 90-day CPAP compliance report, most recent A1C, or any return to duty release forms, if applicable, for your DOT or Non-DOT physical.
- As a courtesy to our DOT drivers, you will receive a copy of your Long Form, Driver’s Certificate, and a card for your wallet.
- Your Driver’s Certificate will also be faxed to the DMV on your behalf, but it is ultimately your responsibility. If the DMV fails to upload the fax, you will receive a notification.
- We collect and process samples for Quest and Labcorp M-F 9:00 am – 4:00 pm and Sat 9:00 am – 11:30 am.
- You will need to present the order or lab slip from your physician. This ensures your provider receives the results. As a collection and processing site, we do not receive results. Please contact your provider if there is a delay. They can reach out to the lab directly.
- Please bring your insurance card and driver’s license.
- Make sure to check with your physician if your blood work requires a fasting blood draw.
- Self-pay labs are available. Please call for pricing.
- To expedite your visit, please download and fill out the VHSL physical form as well as the forms for school physicals here in the State of Virginia.
- If you have a special form, please bring it to your physical, and we would be happy to complete it for you.
- Entry into Virginia schools requires proof of vaccination. The school physical includes this section as well.
- If you have documentation showing proof of vaccination, our providers will go over the records and complete that section of the physical.
- If you do not have the documentation, we will still complete the physical but will omit that section.
- You will have the opportunity to provide documentation before the start of school.
