What you need to know about your Sports / School Physical

  • Currently, we stock VHSL physical forms as well as the forms for school physicals here in the State of Virginia. If you have a special form, please bring it to your physical, and we would be happy to complete it for you.
  • Entry into Virginia schools requires proof of vaccination. The school physical includes this section as well.
  • If you have documentation showing proof of vaccination, our providers will go over the records and complete that section of the physical.
  • If you do not have the documentation, we will still complete the physical but will omit that section. You will have the opportunity to provide documentation before the start of school.
