What you need to know about your Sports / School Physical

• Currently, we stock VHSL physical forms as well as the forms for school physicals here in the State of Virginia. If you have a special form, please bring it to your physical, and we would be happy to complete it for you.
• Entry into Virginia schools requires proof of vaccination. The school physical includes this section as well. If you have documentation showing proof of vaccination, our providers will go over the records and complete that section of the physical. If you do not have the documentation, we will still complete the physical but will omit that section. You will have the opportunity to provide documentation before the start of school.
